Abstract: A obesidade é considerada uma doença complexa, de múltiplas etiologias e capaz de constituir um problema de saúde pública, assim como um dilema clínico importante. A cirurgia bariátrica, apesar de sua natureza invasiva, vem apresentando taxa de sucesso no tratamento da obesidade mórbida, promovendo perda de peso significativa e facilitando sua manutenção. Um dos insucessos da manutenção da perda de peso, promovida por várias terapias de redução de peso, está relacionado com a redução do gasto energético associada à perda de massa magra depois de um período de balanço energético negativo. Estudos relacionados à avaliação da perda de peso promovida pela cirurgia bariátrica registram valores médios de perda de massa magra. Contudo, em longo prazo, o estado nutricional desses pacientes é preocupante devido às alterações fisiológicas promovidas pelas técnicas cirúrgicas. Portanto, o objetivo deste trabalho foi avaliar qualitativa e quantitativamente a ingestão alimentar e a composição da perda de peso dos pacientes no pós-operatório de cirurgia de redução de estômago e suas possíveis implicações nutricionais. Fizeram parte deste estudo todos os pacientes atendidos na Clínica Viver Bem Nutrição (localizada na cidade de Ipatinga-MG) para atendimento nutricional no pré e pós-operatório de cirurgia bariátrica, no período de fevereiro a dezembro de 2006. Foram realizadas avaliações antropométricas, de composição corporal, bioquímicas, dietéticas e dos sinais e sintomas clínicos. Para a avaliação dietética foi utilizado o método recordatório de 24 horas no préoperatório para caracterização dos hábitos alimentares e, mensalmente, para avaliação da evolução da ingestão alimentar. Foram avaliados 25 pacientes no pré- operatório (19 do sexo feminino e 6 do sexo masculino), com idade média de 35,9 ± 11,9 anos e IMC médio de 42,3 ± 5,7 kg/m2. Dos 25 pacientes submetidos à cirurgia, 21 realizaram a cirurgia do tipo Fobi- Capella e 4 a do tipo Scopinaro. As médias da ingestão glícídica, protéica e lípidica foram menores no pós-operatório. Constatou-se ingestão inadequada de cálcio, ferro, vitamina B1 e reduzida ingestão calórica alimentar com valores inferiores ao gasto energético basal. Não houve associação entre a ingestão dietética e os aspectos clínicos e bioquímicos. No primeiro mês de pós- operatório, o sexo feminino apresentou total de perda de peso de 7,5%, sendo 47% de perda de massa muscular; já o sexo masculino apresentou cerca de 10% de perda de peso com 63% de perda de massa magra. Os pacientes atingiram em média peso de 19% mais baixo que o inicial nos três primeiros meses de pós-operatório. Constatou-se aumento dos líquidos extracelulares tanto para o sexo feminino quanto para o sexo masculino. Pode-se concluir que a ingestão alimentar dos pacientes no pós-operatório de cirurgia bariátrica requer uma atenção especial referente a alguns macros (proteínas) e micronutrientes (cálcio, ferro, vitaminas do complexo B), que se encontraram muito aquém das recomendações diárias (RDA) como forma preventiva de evitar possíveis deficiências nutricionais. A cirurgia bariátrica promove uma significativa perda de peso e de massa magra com moderada alterações nos fluidos corporais. Constatou-se redução da taxa metabólica basal justificada pela perda de massa magra neste período.
The obesity is considered a complex disease, of multiple etiologies and capable to constitute a problem of public health, as well as an important clinical dilemma. The bariatric surgery , in spite of her nature aggressive, is presenting success tax in the treatment of the morbid obesity, promoting significant weight loss and facilitating his/her maintenance. One of the failures of the maintenance of the weight loss, promoted by several therapies of weight reduction, it is related with the reduction of the energy expense associated to the loss of thin mass after a period of negative energy swinging. Studies related to the evaluation of the weight loss promoted by the bariatric surgery register medium values of loss of thin mass. However, in long period, the nutritional state of those patient ones is preoccupying due to the physiologic alterations promoted by the surgical techniques. The objective this work was evaluated qualitative and quantitatively the alimentary ingestion and the composition of the loss of the patients' weight in the postoperative of surgery of stomach reduction and their possible nutritional implications. They made part of this study all the patients assisted at the clinic Viver Bem Nutrição (located in the city of Ipatinga-MG) for nutritional service in the pre and postoperative of bariatric surgery, in the period of February to December of 2006. Evaluations anthropometrics were accomplished, of corporal composition, biochemistries, dietary and of the signs and clinical symptoms. For the dietary evaluation the reminding method of 24 hours was used in the preoperative for characterization of the alimentary habits and, monthly, for evaluation of the evolution of the alimentary ingestion.They were appraised 25 patient in the preoperative (19 female and 6 male), with medium age of 35,9 ± 11,9 years and medium IMC of 42,3 ± 5,7 kg/m2. Of the 25 patients submitted to the surgery, 21 accomplished the surgery of the type Fobi-Capella and 4 the one of the type Scopinaro. The averages of the ingestion carbohydrate, protein and fat were smaller in the post- operative. Inadequate ingestion of calcium, iron, was verified vitamin B1 and reduced caloric ingestion to feed with inferior values to the basal energy expense. There was not association between the dietary ingestion and the clinical and biochemical aspects. The first month of postoperative, the feminine sex presented total of loss of weight of 7,5%, being 47% of loss of muscular mass; already the masculine sex presented about 10% of weight loss with 63% of loss of thin mass.. The patients reached weight of 19% lower on average than the initial the first three months of postoperative. Increase of the liquids so much out of cellular was verified for the feminine sex as for the masculine sex. It can be oncluded that the patients' alimentary ingestion in the postoperative of bariatric surgery requests a special attention regarding some nutritious (proteins, calcium, iron, vitamins of the compound B), that you/they were very on this side of the daily recommendations (RDA) as preventive form of avoiding possible nutritional deficiencies. The bariatric surgery promotes a significant weight loss and of thin mass with moderate alterations in the corporal fluids. Reduction of the basal metabolic tax justified was verified by the loss of thin mass in this period.
