Título: Antioxidantes da dieta e sua relação com biomarcadores séricos de estresse oxidativo e inflamação em indivíduos com doença celíaca e sensibilidade ao glúten não celíaca
Dietary antioxidants and their relationship with serum biomarkers of oxidative stress and inflammation in individuals with celiac disease and sensitivity to non- celiac gluten

Abstract: A doença celíaca (DC) é uma enteropatia intestinal imunomediada, na qual a ocorrência de danos ao organismo tem como fator externo a exposição ao glúten nos indivíduos que possuem susceptibilidade genética. Sensibilidade ao glúten não celíaca (SGNC), por sua vez, também pertence ao grupo de distúrbios relacionados ao glúten e sua sintomatologia é desencadeada pelo contato com o mesmo, porém não há presença de anticorpos específicos e/ou atrofia das vilosidades intestinais. Ambas possuem como tratamento dietoterápico dieta livre de glúten. Além da reação imune, parte de toda citotoxidade, bem como as alterações morfológicas e as reações de apoptose são dadas pelo estresse oxidativo, através do desequilíbrio pró-oxidante-antioxidante na mucosa intestinal. Desse modo, o presente trabalho teve como objetivo avaliar o consumo de compostos antioxidantes da dieta e sua relação com biomarcadores de estresse oxidativo e de inflamação em pacientes com DC ou SGNC em orientação nutricional especializada. Metodologia: Trata-se de um estudo do tipo transversal, com 70 participantes (18 homens/ 52 mulheres, idade 35,7 ± 13,8 anos), onde os pacientes com DC e SGNC, são todos atendidos no Programa Pró Celíacos da Universidade Federal de Viçosa. Os participantes foram divididos em três grupos: DC (n=35), SGNC (n= 23) e controle (n= 12). Foram coletados dados sociodemográficos, antropométricos e de saúde. Um questionário de frequência do consumo alimentar também foi preenchido, pelo qual se estimou a capacidade antioxidante total da dieta (CATd). Coleta de sangue em jejum foi realizada para análises de hemograma completo, marcadores metabólicos (colesterol e frações, triglicerídeos, glicose e enzimas hepáticas), enzimas antioxidantes (atividade da superóxido dismutase e glutationa-s-transferase), malondialdeído , marcadores inflamatórios (contagem de leucócitos, linfócitos e proteína-C-reativa – PCR) e capacidade antioxidante total em soro (CATs). Como resultados, a contagem dos eritrócitos no grupo controle foi maior, comparado ao grupo DC (0,60 10 6 /μL ± 0,16 vs. 0,53 10 6 /μL ± 0,15, p=0,012, respectivamente); colesterol total foi maior no grupo SGNC, comparado ao grupo controle (172,44 mg/dL ± 27,94 vs. 149,41 mg/dL ± 24,04, p= 0,040, respectivamente); LDL-c foi maior nos grupos DC e SGNC, comparado ao grupo controle (87,94 mg/dL ± 18,26 vs. 89,66 mg/dL ± 26,19 vs. 69,88 mg/dL ± 21,45, p= 0,037, respectivamente); glicose de jejum foi maior no grupo SGNC, quando comparado ao grupo DC (89,70 mg/dL ± 10,26 vs. 81,09 mg/dL ± 7,08, p= 0,018, respectivamente). Ademais, a atividade de SOD foi maior nos grupos DC (0,50 USOD/mg ± 0,09, p= 0,023, teste Dunnett) e SGNC (0,52 USOD/mg ± 0,08, p= 0,007, teste Dunnett), quando comparado ao grupo controle (0,42 USOD/mg ± 0,05). Em relação à capacidade antioxidante total em soro (CATs), a mesma não se diferiu entre os grupos (F= 2,320, p= 0,110). Da mesma forma, ocorreu para CATd (F= 0,950, p= 0,392), porém, a CAT proveniente do grupo de pães foi superior no grupo controle, comparado aos grupos DC e SGNC (0,77 mmol ± 0,43 vs. 0,36 mmol ± 0,25 vs. 0,43 ± 0,30, p=0,001, respectivamente). A ingestão dos minerais selênio (F= 6,604, p= 0,003) e zinco (F= 4,192, p= 0,020) também foram superiores no grupo controle e semelhantes entre DC e SGNC. Encontrou-se ainda correlação negativa entre atividade de SOD e ingestão de cobre (r= - 0,304, p= 0,036), zinco (r= -0,366, p= 0,011), selênio (r= -0,352, p= 0,014) e vitamina E (r= -0,352, p= 0,015). E correlação positiva entre CATs e CATd (r= 0,351, p= 0,015 ) e zinco (r= 0,495, p= 0,000). De modo interessante, o tempo de acompanhamento nutricional no programa Pró Celíacos se correlacionou positivamente com o consumo de frutas (r= 0,277, p= 0,049) e hortaliças (r= 0,287, p= 0,041) e negativamente com o consumo de doces (r= -0,280, p= 0,046). Finalmente, o tempo de acompanhamento nutricional também teve uma correlação negativa com as concentrações de colesterol total (r= -0,407, p= 0,010) e LDL-c (r= -0,435, p=0,006). Como conclusão, nosso estudo indica que apesar da DC associar-se com processos imunológicos e estresse oxidativo como mostrado na literatura, pacientes com doença celíaca ou sensibilidade ao glúten não celíaca em orientação dietético-nutricional especializada, não apresentam diferença em relação aos marcadores de inflamação, de estresse oxidativo e atividade da GST em comparação ao grupo controle. E apesar do consumo de alguns nutrientes com capacidade antioxidante serem maior no grupo controle, o maior tempo de acompanhamento nutricional promove maior conscientização do consumo das frutas e hortaliças.
Celiac disease (DC) is an immune-mediated intestinal enteropathy, in which the occurrence of damage to the body has as an external factor the exposure to gluten in individuals who have genetic susceptibility. Sensitivity to non-celiac gluten (SGNC), in turn, also belongs to the group of disorders related to gluten and its symptomatology is triggered by contact with it, but there is no presence of specific antibodies and / or atrophy of intestinal villi. Both have diet therapy as a gluten-free diet. In addition to the immune reaction, part of all cytotoxicity, as well as morphological changes and apoptosis reactions are given by oxidative stress, through the pro-oxidant-antioxidant imbalance in the intestinal mucosa. Thus, the present study had as objective to evaluate the consumption of antioxidant compounds of the diet and its relation with biomarkers of oxidative stress and inflammation in patients with CD or SNCG in specialized nutritional orientation. Methodology: This is a cross-sectional study with 70 participants (18 men / 52 women, age 35.7 ± 13.8 years), where patients with CD and SNCG are all treated in the Pró Celíacos Program of the University Federal of Viçosa. Participants were divided into three groups: CD (n = 35), SNCG (n = 23) and control (n = 12). Sociodemographic, anthropometric and health data were collected. A food intake frequency questionnaire was also filled out, which estimated the total antioxidant capacity of the diet (CATd). Fasting blood collection was performed for analyzes of complete blood count, metabolic markers (cholesterol and fractions, triglycerides, glucose and liver enzymes), antioxidant enzymes (superoxide dismutase activity and glutathione-s-transferase), malondialdehyde, inflammatory markers Leukocytes, lymphocytes and C-reactive protein - CRP) and total antioxidant capacity in serum (CATs). As a result, the erythrocyte count in the control group was higher, compared to the DC group (0,60 10 6 / μL ± 0,16 vs. 0,53 10 6 / μL ± 0,15, p = 0,012, respectively); total cholesterol was higher in the SNCG group, compared to the control group (172,44 mg / dL ± 27,94 vs. 149,41 mg / dL ± 24,04, p = 0,040, respectively); LDL-c was higher in the CD and SNCG groups, compared to the control group (87,94 mg / dL ± 18,26 vs. 89,66 mg / dL ± 26,19 vs. 69,88 mg / dL ± 21.45 , P = 0,037, respectively); fasting glucose was higher in the SNCG group, when compared to the CD group (89,70 mg / dL ± 10,26 vs. 81,09 mg / dL ± 7,08, p = 0,018, respectively). In addition, SOD activity was higher in the CD groups (0,50 USOD / mg ± 0,09, p = 0,023, Dunnett's test) and SNCG (0,52 USOD / mg ± 0,08, p = 0,007, Dunnett's test ), when compared to the control group (0,42 USOD / mg ± 0,05). Regarding the total antioxidant capacity of the serum (CATs), it was not different between the groups (F = 2,320, p = 0,110). In the same way, it occurred for CATd (F = 0,950, p = 0,392), but CAT from the group of breads was superior in the control group, compared to the groups CD and SNCG (0,77 mmol ± 0,43 vs. 0,36 mmol ± 0,25 vs. 0,43 ± 0,30, p = 0,001, respectively). Selenium mineral intake (F = 6,604, p = 0,003) and zinc (F = 4,192, p = 0,020) were also higher in the control group and similar between CD and SNCG. There was negative correlation between SOD activity and copper intake (r = -0,304, p = 0,036), zinc (r = -0,366, p = 0,011), selenium (r = -0,352, p = -0,352, p = 0,015). And positive correlation between CATs and CATd (r = 0,351, p = 0,015) and zinc (r = 0,495, p = 0,000). Interestingly, nutritional monitoring time in the Pro Celíacos Program correlated positively with fruit consumption (r = 0,277, p = 0,049) and vegetables (r = 0,287, p = 0,041) and negatively with candy consumption (r = -0,280, p = 0,046). Finally, nutritional monitoring time also had a negative correlation with total cholesterol concentrations (r = - 0,407, p = 0,010) and LDL-c (r = -0,435, p = 0,006). As a conclusion, our study indicates that although CD is associated with immunological processes and oxidative stress as shown in the literature, patients with celiac disease or sensitivity to non-celiac gluten in specialized dietary-nutritional orientation do not present difference in relation to markers of inflammation, of oxidative stress and GST activity in comparison to the control group. And although the consumption of some nutrients with antioxidant capacity are higher in the control group, the longer nutritional monitoring promotes greater awareness of the consumption of fruits and vegetables.
