dc.description.abstractA avaliação de programas, serviços e tecnologias na saúde, passa por um processo de expansão e diversificação conceitual e metodológica, sendo considerado um importante instrumento de apoio às decisões necessárias à dinâmica dos sistemas e serviços de saúde visando a implementação de políticas. Neste sentido, este estudo teve como objetivos: avaliar a qualidade da atenção à saúde materno-infantil do Programa de Saúde da Família (PSF) do município de Teixeiras MG, dentro de uma perspectiva centrada no cuidado à saúde e na integralidade; discutir os desafios e limites do PSF como estratégia de reorientação do modelo assistencial e como produtor de cuidados; analisar de forma reflexiva os métodos e modelos de avaliação em saúde em uma perspectiva do cuidado pensado e praticado segundo as necessidades e demandas do usuário; analisar as concepções e representações sociais femininas (das mães) do processo saúde-doença. O desenho do estudo foi observacional de corte transversal com abordagem quanti-qualitativa, tendo participado da pesquisa 161 mães de crianças menores de dois anos, representando 94,7% deste grupo populacional, 33 gestantes, representando 82,5% deste grupo populacional, 35 profissionais de saúde atuantes no PSF, representando 97,2% dos profissionais da atenção básica do município. Na avaliação da atenção à saúde da criança e da gestante pelo PSF, de acordo com as proposições de Donabedian (1990) que relaciona as dimensões de estrutura, processo e resultado, utilizou-se um sistema de escores específicos para análise de tais dimensões e seus respectivos atributos, classificando o PSF nos cenários incipiente, intermediário e avançado se obtivesse entre 0,0 e 33,3%; 33,4 e 66,6%; 66,7 e 100,00%, respectivamente. Com relação às representações sociais utilizou-se a Técnica de Análise de Conteúdo (Minayo, 1992), construindo atrativos semânticos e indicadores de saúde na busca de expressar a representação do processo saúde-doença pelas mães. Utilizou-se questionários semi estruturados na coleta das informações, aplicados por um único entrevistador devidamente treinado. Os resultados indicaram que o PSF na atenção a saúde das crianças e gestantes classifica-se como um cenário intermediário, caracterizado por uma atenção fragmentada, mas com alguns avanços na organização da atenção para estes grupos. Na atenção a saúde das crianças, foram consideradas incipientes as instalações físicas, a qualidade do cuidado nas ações direcionadas ao controle da diarréia e das infecções respiratórias, à participação comunitária e a intersetorialidade. Os avanços estiveram relacionados ao acolhimento realizado pelo PSF e a resolubilidade. Na atenção a saúde das gestantes os atributos caracterizados como incipientes foram as instalações físicas, as atividades educativas, a intersetorialidade e a participação comunitária; os atributos caracterizados como intermediários foram os recursos materiais, a avaliação clínica, o cartão da gestante, a referência e contra-referência e a resolubilidade, e os atributos avançados foram o acolhimento e a acessibilidade. As representações sociais das mães expressam uma visão de saúde-doença pluralista, das mais holísticas às mais biomédicas; das mais relacionais às mais individuais. Um dado interessante encontrado no estudo, foi a apresentação da busca da felicidade como um conceito de saúde pelas mães, retomando aspectos filosóficos aristotélicos e spinozistas. Não obstante, a representação da saúde como ausência da doença aparece também como ponto importante no relato das mães. Considerando que os construtos são sociais e não individuais e recebem grande influência da cultura local e macro social, o modelo assistencial hegemônico vigente fornece ainda fortes referenciais para interpretar tais experiências. O PSF na atenção a saúde das crianças e das gestantes em suas práticas de saúde continuam centradas nos procedimentos e com sua forma de organização desarticulada e isolada, influenciadas pelo modelo hegemônico biomédico que corrompe suas ações e despreza as práticas de prevenção das doenças e promoção da saúde, funções estas inerentes do PSF. A superação do modelo biomédico (flexneriano), ainda dominante nas práticas de saúde locais, ações e serviços, somente se concretizará com a participação de todos os atores sociais envolvidos nas ações e serviços de saúde. Esta participação poderá ser viabilizada na medida em que os gestores e profissionais de saúde compreendem o papel ativo dos usuários e de suas representações no planejamento, na formulação e na avaliação das ações e atividades desenvolvidas pelo programa, fomentando a saúde em uma base ética e política.pt_BR
dc.description.abstractThe evaluation of programs, services and technologies of the health go through an expansion process and conceptual and methodological diversification, being considered an important support instrument to the necessary decisions to the dynamics of the systems and services of health seeking the implementation of the politics. This study had as objectives: to evaluate the quality of the attention to the maternal-infantile health for the Program of Health of the Family (PSF) of the municipal district of Teixeiras - MG, inside of a perspective centered in the care to the health and in the integralities; to discuss the challenges and limits of PSF as strategy of reorientation of the assistance model and as producing of cares; to analyze in a reflexive way the methods and evaluation models in health in a perspective of the thought care and practiced according to the needs and demands of the usury; to analyze the conceptions and feminine social representations (of the mothers) of the process health-disease. The drawing of the study was observational of traverse cut with quantity -qualitative approach, having participated in the study mothers of smaller children two years old (161), representing 94.7% of this population group, 33 pregnant, representing 82,5% of this population group, 35 active professionals of health in PSF, representing 97,2% of the professionals of the basic attention of the municipal district participated of this research . In the evaluation of the attention to the child's health and of the pregnant woman for PSF, in agreement with the propositions of Donabedian (1990) that relates the structure dimensions, process and result, a system of specific scores was used for analysis of such dimensions and their respective attributes, classifying PSF in the incipient sceneries, middlemen and advanced if it obtained between 0,0 and 33,3%; 33,4 and 66,6%; 66,7 and 100,00%, respectively. Regarding the social representations the Technique of Analysis of Content was used (Minayo, 1992), building semantic attractions and indicators of health in the search of expressing the representation of the process health-disease for the mothers. Semi-structured questionnaires were used in the collection of the information, applied for a single interviewer properly. The results indicated that PSF in the attention to the children's health and pregnant women were classified as intermediate scenery, characterized by a fragmented attention, but with some progresses in the organization of the attention for these groups. In the attention to the children's health, the physical facilities, the quality of the care in the actions addressed to the control of the diarrhea and of the breathing infections, were considered incipient to the community participation and the intersectoriality. The progresses were related to the reception accomplished by PSF and the resolubility. In the attention to the pregnant women's health the attributes characterized as incipient were the physical facilities, the educational activities, the intersectoriality and the community participation; the attributes characterized as middlemen were the material resources, the clinical evaluation, the pregnant woman's card, the reference and against-reference and the resolubility, and the advanced attributes were the reception and the accessibility. The mothers' social representations expressed a pluralist vision of health-disease, of the more holistic to the most biomedical; of the more relates to the most individual. An interesting date found in the study was the presentation of the happiness search as a concept of health for the mothers, retaking Aristotelian philosophical aspects and spinozists. The representation of the health as absence of the disease also appears about important point in the mothers' report. Considering that the constructs is social and no individual and they receive great influence of the local culture and social macro, the assistance model effective hegemonic still supplies strong references to interpret such experiences. PSF in the attention the children's health and of the pregnant women in their practices of health continue centered in the procedures and with their form of disjointed and isolated organization, influenced by the biomedical hegemonic model that it corrupts their actions and it despises the practices of prevention of the diseases and promotion of the health, functions these inherent of PSF. The superation of the biomedical model (flexneriano), still dominant in the local practices of health, actions and services, it will only be rendered with the social actors' participation of all involved in the actions and services of health. This participation can be made possible when the managers and professionals of health understand the users' active paper and of their representations in the planning, in the formulation and in the evaluation of the actions and activities developed by the program, fomenting the health in an ethical and political base.eng
