Parâmetros organizacionais e caracterização do serviço de cirurgia ambulatorial da Universidade Federal de Viçosa

Abstract

A cirurgia de caráter ambulatorial é aquela realizada sob qualquer tipo de anestesia, sem internação hospitalar, com permanência do paciente no serviço que não excede 24 horas. As intervenções podem ser realizadas de forma adequada, rápida e segura, possibilitando ao paciente retornar rapidamente ao domicílio e às suas atividades cotidianas. O presente estudo teve como objetivo avaliar como ocorreu a evolução organizacional e de produtividade do serviço de cirurgia ambulatorial vinculado à Universidade Federal de Viçosa (UFV). Para tanto, buscou estudar o impacto deste ambulatório no cuidado da saúde de pacientes do Sistema Único de Saúde da Microrregião de Viçosa analisando, especificamente, a capacidade de organização do ambulatório de manter o acompanhamento necessário do paciente, bem como a de solucionar problemas num curto prazo e, por fim, elaborar uma cartilha de divulgação dos serviços prestados no ambulatório de pequenas cirurgias, visando orientar pacientes e secretarias de saúde da microrregião de Viçosa. A pesquisa foi realizada no ambulatório de pequenas cirurgias da UFV. Para cumprir os objetivos propostos, foram analisados 368 prontuários de pacientes atendidos no serviço de cirurgia ambulatorial no ano de 2018. Com o auxílio de uma planilha elaborada no Microsoft Office Excel, foram coletados os dados, que passaram por análise com auxílio do software Statistical Package for the Social Sciences (SPSS®). O serviço de cirurgia ambulatorial da UFV funcionava conveniado com o Consórcio Intermunicipal de Saúde de Viçosa (CSMIV), atendendo 10 municípios da microrregião, sendo que o município sede representava mais de 40,0% da demanda de atendimentos. A maior parte dos pacientes era encaminhada pelas UBS, conforme preconizado pelo SUS. A maior parte dos atendimentos foi resolutiva, sendo necessário reencaminhar pequena porcentagem de pacientes a outros serviços. A maioria dos procedimentos realizados se enquadrou em exérese de lesões superficiais. Após o procedimento, quando necessário, os pacientes eram orientados a encaminhar a peça cirúrgica para exame anatomopatológico e, posteriormente, retornar com o resultado do exame para que fossem tomadas as medidas necessárias. O convênio entre UFV e CISMIV se mostrou uma solução eficiente e resolutiva para atendimento da demanda de cirurgia ambulatorial, apesar de apresentar fragilidades de ordem organizacional e funcional. Nesse sentido, buscando sanar os problemas de informação da população beneficiária, esta dissertação apresenta como produto final uma cartilha ilustrada com diversas orientações para os pacientes.

Outpatient Surgery is performed under any type of anesthesia, without hospitalization, with patient stay in the service and without exceeding 24 hours. The interventions can be performed adequately, in a fast and safe way, allowing the patient to return quickly to the home and to their daily activities. The present study had as objective to evaluate how the organizational and productivity evolution of the ambulatory surgery service linked to the Federal University of Viçosa occurred. In order to do so, the purpose of this study was to investigate the impact of this outpatient clinic on the health care of patients from the Unified Health System in the Micro- region of Viçosa, specifically analyzing the ambulatory organization's capacity to maintain the necessary patient follow-up, as well as to solve problems in a short term, and finally, the ability to prepare a booklet for the dissemination of services provided in the small surgery outpatient clinic, aiming to guide patients and health secretariats of the Viçosa microregion. The research was performed at the UFV's small surgeries clinic. In order to fulfill the proposed objectives, 368 medical records of patients attended at the outpatient surgery service in the year 2018 were analyzed. Using a spreadsheet developed in Microsoft Office Excel, data were collected and analyzed with the aid of the Statistical Package for the Social Sciences (SPSS®) software. The UFV's outpatient surgery service was contracted with the Inter-municipal Health Consortium of Viçosa, serving 10 municipalities in the microregion, with headquarters representing more than 40.0% of the demand for care. Most of the patients were referred by the UBS, as recommended by SUS. The majority of the visits were resolutive, and only a small percentage of patients were referred to other services. Most of the procedures performed were involved in superficial lesion removal. After the procedure, when necessary, the patients were instructed to refer the surgical specimen for anatomopathological examination and, later, to return with the result of the examination so that the necessary measures were taken. The agreement between UFV and CISMIV proved to be an efficient and resolutive solution to meet the demand for outpatient surgery, despite presenting organizational and functional weaknesses. In this sense, seeking to solve the information problems of the beneficiary population, this dissertation presents an illustrated booklet with several orientations for patients as a final product.
